Silent Witness Report Form

Silent Witness is a program that allows for the anonymous reporting of suspicious, unusual and/or criminal activity that occurs on the University Park Campus, Health Sciences Campus and surrounding communities. Submitted information will be reviewed by our Investigative staff and appropriate action will be taken.

If you observe suspicious and/or unusual behavior or witnessed a crime on campus or in your apartment/dorm room and you would like to report it, please fill out the form below.
