As a member of USC's faculty or staff, you can take ten (10) weeks of maternity leave if you are enrolled in USC's short-term disability plan. You can begin your ten-week leave as early as two weeks prior to your delivery date and up to the day of delivery.
HOW TO APPLY FOR MATERNITY LEAVE
- Initiate a claim by calling Sedgwick at (800) 495-2315. For more information refer to the How to File a Claim section.
- Complete the Maternity Disability Request (MDR) form. We suggest that you send the form one month before your expected delivery.
- Fax the form to (213) 740-7305. Follow up by calling (213) 740-5875 to make sure the form has been received.
WHAT IF THE MDR FORM IS NOT SUBMITTED BEFORE MY DELIVERY DATE?
In the case of normal pregnancies, your leave will follow California State guidelines, which allow for six weeks off for post-normal delivery and eight weeks for post C-section delivery.
