| When an employee in your department is injured on the job, you are responsible for three things: |

- Find out what happened – You are our first line of defense, whether it is to find unsafe working conditions or practices, or fraud. It is important to us that you investigate incidents thoroughly. Take statements from the injured worker and any witness to find out how the accident occurred. Follow up to make sure the unsafe conditions are corrected to prevent future occurrences. If an injury seems questionable, call our office at (213) 740-6205 or email us for further instructions.

- Make sure that the proper forms reach our office. The Supervisor’s report of Injury can be completed via our online system, http://srm.usc.edu/rmcsapps/login.htm. or faxed to us at (213) 740-7305. If you complete the form online, please fax the employee’s claim to us.

- Make sure the employee goes to the proper medical facility, UPC employees should go to the Student Health Center and HSC employees should go to the Internal Medicine (HCCII).

| Injuries to Non-USC Employees: |
1. Unpaid Students who are injured in your lab , Workers' Compensation does not apply. In this situation, the student should be sent to the Student Healt Center (UPC) and Family Practice Student Health Center (HSC). |
2. Temporary Agency Employees - contact the Temporary Agency for Workers' Compensation procedures.
