Report all workplace injuries to your Supervisor immediately.

Supervisor must complete:

  • Supervisor's Report of Injury

Employee must complete:

  • Employee's Claim for Workers' Compensation Benefits
 
You should take a copy of the Supervisor’s Report of Injury with you to your first doctor’s visit. The above forms can be obtained from your Supervisor, Home Department Coordinator, or Workers’ Compensation Office, or downloaded from our website: http://srm.usc.edu/workerscomp.
 
Prompt reporting eliminates delays in benefits to employees.
 
Timing is everything: State law requires that all (major and minor) injuries be reported within twenty-four (24) hours.
 
For Injuries that occur over the weekend or after regular business hours (Monday thru Friday, 8:30 AM to 5:00 PM) reports should be completed and sent to our office the next regular working day after the accident. Please call us to let us know about the injury and that the report is on the way.
