How does an employee file for PFL? The PFL claim form from the State will be available in 2004. Sedgwick CMS will be prepared to intake employee PFL claims by July 1, 2004 by telephone at 1-800-495-2315 or on their website www.vpainc.com.
How long does an employee have to file for PFL? An employee is required to file within 41 days from the first compensable day.
Is a medical certification required? Yes, there must be medical certification from the treating physician. The certification will require the diagnosis, date leave is required, probable duration, and statement that the health condition requires the employee's participation.
What is required for bonding or adoption? Documentation of the birth, adoption, or foster care placement will be required, such as birth certificates, completed hospital or birthing center documents, or Department of Social Services forms for adoption or placement.
Is a release of information going to be required to get the diagnosis information from the family member's physician? Yes, it is likely that the employee or the administrator will have to provide a release of information to the family member's physician to get the certificate information.
Special Instructions for Home Department Coordinators:
The Disability/Workers' Compensation Online System can be used to complete DDN's for PFL claims. The web address is http://srm.usc.edu/rmcsapps/login.htm. Completing a DDN for a PFL claim is very similar to completing a DDN for a Disability claim. The only difference is a few added questions regarding the claim for PFL.
