Silent Witness Report Form

Silent Witness is a program to allow for the anonymous submission of suspicious, unusual and/or criminal activity that occurs on the University Park Campus, Health Sciences Campus and/or surrounding communities. Submitted information will be reviewed by our Investigative staff and appropriate action will be taken as warranted.

If you observe suspicious and/or unusual behavior or see a crime occurring on campus or in your apartment/dorm room and you would like to report it, please fill out the form below.
