paid disability leave you may also be eligible for unpaid leave under the
Federal Family and Medical Leave Act (FMLA) and the California Family Rights
Act (CFRA). For

| The university provides 10-weeks of paid maternity
disability leave to all faculty. The 10-week maternity disability leave runs concurrent with FMLA leave and the university's 10-week Parenting Leave. A faculty member may not take a 10-week maternity disability leave and then 10-weeks of Parenting Leave. However, a faculty member is eligible for 6-weeks of Paid Family Leave for child bonding (Please see the Paid Family Leave section of this website). The 10-weeks of paid maternity disability leave can begin on the day of delivery, or 2-weeks before
delivery and 8-weeks after delivery, or some other commutation of time. This 10-week period can be extended
with a physician's certification if medical complications exist. To begin
a paid maternity disability leave a faculty member can call our third-party administrator,
Sedgwick CMS, at 1-800-495-2315 or visit their website at www.vpainc.com
. Under the heading "Employee" click on "File a Claim On-line" and follow
the instructions. Benefits will be paid according to which disability plan
the faculty member is enrolled in. If your disability benefits are less than 100% of your usual and customary salary, your department will "top up" the disability benefits to 100% of your salary. Under no circumstances will you receive more than 100% of your usual and customary salary. All information regarding the USC disability

| Full time faculty, male or female,
can take a non-medically certified 10-week leave under the following circumstances: |

A father can take "paternity" leave within six months
of the birth event. |
|
Both mother and father can take leave in the event of an
adoption of a child under the age of 6 years. This must also be taken within
6-months of the adoption.
(NOTE: Paid Family Leave may also be taken to bond with an adopted or foster care-placed child). |
| To secure this type of leave a Full Time Faculty Parenting Leave form must be completed. The form can be
obtained from your Home Department Coordinator, the Provosts' Office, or
the Disability website (Faculty Family Leave Request). The form must contain the signature of the faculty
requesting the leave, plus the appropriate Chair and Dean, as well as the
Provost's Office. This type of leave will be paid by the department and
reimbursed by the Disability Office. The account number to reimburse
must be included on the form. Please note: Sponsored Project Accounts
(SPA) cannot be used to pay maternity/paternity leave by a department .
