Supervisors and Home Department Coordinators Responsibilities
What Should Supervisors Do?
1 Keep in touch with employees; check on their well-being.
2 Make sure that they have received FMLA notification letter.
3 Remind them of the importance of staying in touch with you, as well as with Sedgwick, especially if there is any change in their disability status (for instance, a return to work date).
   
What Should Home Department Coordinators Do?
1.

Make sure the employee knows how to file a claim for benefits, employees should contact Sedgwick at

(800) 495-2315 or Employment Development Department if they signed up for the State Plan.

2. Complete Department Disability Notice (DDN) form.
3.

Zero out pay line so that the employee will not be paid by your department.  

4. When the employee returns to work, notify the Disability Office, request Access and forward Data form.
