with Sedgwick, especially if there is any change in their disability status (for

Make sure the employee knows
how to file a claim for benefits, employees should contact Sedgwick at
(800) 854-6188 or Employment
Development Department if they signed up for the State Plan. |

Complete Department Disability Notice (DDN) form. |

Zero out pay line so that the
employee will not be paid by your department.

When the employee returns to work, notify the Disability Office, request
Access and forward Data form. |
